Service Requested

I hereby authorize appropriate personnel to enter my unit to make the necessary repairs. If the service requested is determined to be caused by the tenant's negligence, the tenant agrees to pay for said service. Tenant will also be charged if refused, access to the unit is denied to maintenance personnel, or if pets are unrestrained. The minimum service charge/rate will be $60.00 per hour (parts additional).
